It has come to my attention that it is once again time for your full body physical. As a physician yourself, you should be aware of the importance of keeping to the required schedule. Please check the option which bests fits your schedule.

______ Abbreviated dinner of beer and pizza, followed by full body exam.
______ Cut to the chase - full body exam with a few extra "frills".

You certainly have a way with words. Normally, I would check option number 1, but as you stated, it is important to have these exams on a regular basis, and as I have not had one for . . . hmmm! - three or four days - I'm feeling the need for one immediately. Therefore, I choose option 3 to be scheduled for this evening
